Indications Include

1. Isolated areas of severe bone loss involving an individual root
2. Fractured root
3. Failure of endodontic therapy or inoperable/calcified canals
4. Bone resorption involving the furcation of multi‐rooted teeth
5. Close interdental root proximity making plaque control impossible (C)
6. Extensive root exposure contraindicating new attachment procedures
7. Root or furcation perforation
8. Root resorption (internal and/or external)
9. Extensive caries in the furcation/roots (B)

Contraindications Include

1. Advanced bone loss and secondary occlusal trauma
2. Individual roots fused or in close proximity
3. Remaining root cannot be restored and/or endodontically treated
4. Poor root form or length of retained roots
5. Apical location of furcation area resulting in severely compromised bone support of remaining root(s)(D)
6. Inability to create a good post‐surgical mucogingival environment with an adequate zone of attached gingiva and vestibular depth
7. Evidence of poor oral hygiene
8. When interdisciplinary therapy (endodontics, periodontics, restorative) is not possible due to patient’s finances or medical history
