二、SURVEY REPORT (II)(20 分) The following points are given as a guide to the contents required in the survey report: 1. The date/s and locations of the survey/s. 2. The name and designation of all persons present at the survey/s. 3. Copies of any notes of protest and extensions made if any. Details of the weather during loading, the voyage and discharge. 4. Description of the shipment, including place of origin and a description of the damage. 5. Details of loading and discharge. 6. Details of the survey conducted, with a full description of the damage or shortage and a description of the packaging.7. Details of transit, prior to loading and after discharge. 8. The name and address of the shipper and consignee. 9. Appropriate extracts from the ship’s log book. 10 Details of any liability of third parties and details of any claims made.

三、SHIP SURVEY(15 分) The following damage was found: 1. Tank top 12 m abaft bulkhead and 7 m inboard of port shell plating shallow indent 100 × 120 × 10 mm. 2. 4th frame for’d of bulkhead - flange bent in two places over 400 mm length. 3. After bulkhead - numerous indentations (say 100) to max. depth 20 mm. 4. Port aft and for’d bulkhead sounding pipes - 2 protective straps torn off each end. 5. Mast house between Nos. 2 & 3 hatches - numerous dents, including sharp dent to 15 mm depth and 3 new scars 20 × 20 × 5 mm.

四、CARGO SURVEY(20 分) Underwriters for the cargo may call for a warranty survey when high value cargo is to be shipped. The intention here is to protect the underwriters’ interests by ensuring that the carrier observes good practice, as stipulated by the surveyor. Shippers may call for surveys simply to protect their products from poor stowage, or to provide evidence of quality of the cargo at export. The pre-shipment surveys can be divided into two groups - those centered on the cargo and those centered on the compartment.
