後來查了課本：

isosorbide dinitrate 代謝成

2-mononitrate (半衰期2hr) 、5-mononitrate(半衰期5hr)

雖然肝臟代謝快，生體可用率30%，但是活性代謝物，

所以作用時間較長，口服可以到半小時


解題的想法是這樣

(A)glyceryl trinitrate 線性 三個NO2
(B)isosorbide dinitrate 環狀 兩個NO2
(C)erythrityl tetranitrate 線性 四個NO2
(D)pentaerythritol tetranitrate 線性 四個NO2

因為他要代謝物作用時間比原來長

表示代謝物比較不容易被代謝

環狀→線狀→NO2 就是比別人多了一步

Isosobride mononitrite 可避免吸收不穩定及首度效應、半衰期更長

心絞痛
1. 穩定性：心作功太多，HR太快致缺血，首選BB
2. 不穩定性：血栓，首選抗血小板藥(=治動脈血栓)(1)Aspirin(TXA2抑制劑)(2)Clopidogrel(ADP抑制劑)
3. 變異性：冠狀動脈痙攣，首選CCBS(但Nifedipine少用，因抑制血管大於心，所以BP下降致反射性HR上升)

PS. Amyl nitrite脂溶性最大，Onset最快
