1.1 早上 10:00 左右,貨艙停止貨物作業,艙口像往常一樣被妥善蓋好。大約在下午 15:00,為了在第五貨艙裝載更多的貨物,碼頭工人 ABC 君,由大約27 英尺的高 度的艙口圍緣,跌落到貨艙底部,位置在軸隧道頂部右邊位置,當場死亡。具體情 況請參考附圖。

1.2 根據 XX 日的租船合約,延滯費將從6月30日14:30起算,一直持續到船舶卸貨 完畢時止,每天以5,000 英鎊計算,不足一天部份按比例計算。

 In July last year, we announced our intention to withdraw from the production of paper charts. Following consultations with the Maritime and Coastguard Agency and national user groups, we set a target date of late 2026 to complete the process. This was subject to the development of digital solutions for those remaining users of ADMIRALTY Standard Nautical Charts (SNCs) and Thematic Charts, ensuring that they have viable, official alternatives, as well as meeting the technical and regulatory steps required to achieve this. These conversations have highlighted a number of important transnational and regulatory factors that need further consideration, and we are grateful to every organization that has engaged in this process. It has become clear that more time is required to address the needs of those specific users who do not yet have viable alternatives to paper chart products, so we will continue to provide a paper chart service until at least 2030.

Impact on our paper chart service

In managing the service, it will be important that we have a deeper understanding of the changing value of paper charts to the mariner. While we recognize there is a move towards digital charting, we also understand that this is occurring at different rates in different parts of the world. Therefore, we are increasing efforts to further our knowledge of the use of paper charts on a global scale, and will use this to direct the service to meeting the needs identified.

Finding of Concern

It is recommended that vessel owners and operators ensure and promote crew awareness of policies regarding the duties and obligations of officers on watch for the safety of the ship, even when a pilot is embarked. International Maritime Organization (IMO) Resolution A.960(23) highlights that efficient pilotage largely depends upon the effectiveness of communications and information exchange between the pilot, master, and bridge personnel regarding navigational procedures, local conditions, and ship's characteristics.

The IMO advises that this information exchange should be a continuous process that is generally ongoing for the duration of the pilotage. The IMO further emphasizes that Masters and bridge officers have a duty to support the pilot and ensure that his or her actions are monitored at all times. It is essential that these procedures are not only reflected in the vessel's Safety Management System but also regularly used and practiced during transits with pilots on board.

 3.1 0900 時,測得領港站雷達訊標(A),方位 295 度,距離 5.16 海里處,設定航向115 度往關島(Guam)開,船速變化依船長伸令調整。

3.2 1000 時,測得查母嶼(Char-Mo Is.)燈塔方位 236度,距離3.14 海裡,船速 11.7節, 轉向至180度。

3.3 1700 時,遵守美國聯邦法(CFR Code) 第 33 冊 164.25 款要求,到港前測試主機、 舵機及相關航儀等設備,確認所有情狀完好。

3.4 1400~1600時,舉行放下救生艇海上救生、貨艙滅火、密閉空間救援應急操演, 並依到港前船舶保全程序,確認無偷渡客情事。

3.5 夜令:若遭遇起霧,必需依據避碰規則保持適當瞭望,如有任何問題,請勿遲疑, 立即叫我。

3.6 0000~1400,為減少惡劣天候對船舶衝擊及保護貨物,採取減速,並依當時海浪狀 況需要改變航向,避免橫搖及甲板上浪。海事報告及其他通告
