ABC輪的情況及需求協助支援與相關的ISM附件名目附註等訊息。 事故概述:15分鐘的危機過程13:57 時33秒:根據該輪的加減速情況,推測該輪引航員是在距離泊位6.5海浬的“Busan New Port Masan”登輪點位置登輪的。登輪後,船舶航速由7節多增加到9節多。14:34時53秒:進防波堤時,航速為9.5節,距離泊位的垂直距離約為1海裡,航行在左側標與右側標識島嶼的專用標誌中間的位置,此處連接水域寬600多米。14:37 時03秒(船位1):航速8.6節,距離碼頭垂直距離 900米左右。在本船位之前該輪開始右轉。14:39時51秒(船位2):航速6.8節,定位點與泊位距離約450米,因入泊角度較大,根據其船艏至駕駛台距離142.2米推測,此時船艏距離泊位在泊船的法線距離約為300多米。14:42時32秒(船位3):航速5.4節,航向78度,定位點與泊位距離約150米。考慮入泊角度、靠泊船寬,推測此時與距離在泊船的法線距離不足百米。14:44時32秒(船位4):航速6.1節,航向87度,定位點與泊位距離約80米。考慮入泊角度、在泊船寬,推測此時與距離在泊船的法線距離不足半倍船寬。14:47時52秒(船位5):航速6.2節,航向83度,定位點與泊位距離約60米。14:49時12秒(船位6):航速5.8節,航向84度,定位點與泊位距離約40米。發生碰撞。事故概述:風/流訊息。 A. 碼頭/橋吊損壞部分:a)一橋吊倒塌在船艉.,全損.b)另一架橋吊傾斜變形.c)有兩橋吊外部損壞及脫軌,無法操作.d)隔壁另兩橋吊外部損壞,有一可操作.e)總計六個橋吊,五個無法操作,有一可操作.f)2000米碼頭中長有580米無法行使正常商業作業B. 船舶損壞有部分:a)嚴重的船舶左舷船體水線上及甲板下的走道.b)船艉繫索架/螺旋槳/左舷駕駛台船翼.C. 碼頭工作人員有一人輕傷/無油水汙染

(1)(Root Cause) (英翻中)A. High approach speed.a. Uncontrollable speed.b. Very less reaction time after the turn.c. Lessened effect of tug's pull due to high FWD speedd. No effect of Bow ThrusterB. Failure to recognise vessel's condition.a. Light condition, partly submerged Rudder and propeller providing less steering effect & lesser ROT. Propeller immersion: approx. 60%, Rudder Immersion: Partial corresponding to propeller immersion.b. Light condition provides large exposed transom and less parallel body & quick response to helm and engine, hence very careful approach to berth is required.C. Failure to recognise local conditions.a. Land breeze effect resulting on-shore wind is expected during berthing at around noon time.b. Flood tide contributed the lesser effect of the Rudder and more speed.D. From the beginning of the turn to a substantial period 20 Degrees of STBD helm used, resulted in less ROT, a quicker turn could have given the vessel better clearance from the moored vessels and the berth.E. Failure to challenge Pilot's intention.a. High approach speed was not challenged by Bridge team even when the pilot's panic was noticed well in advance.b. Not choosing the other approach from south of the Island to have better control on a flood tide.

To gauge the demand of Hong Kong registered ships seeking assistance from Chinese naval warships, ships scheduled to transit the Gulf of Aden are reminded of the need to submit a ''ship movement report" via e-mail to the Hong Kong Marine Emergency and Maritime Rescue Coordination Centre (HKMRCC). The email address of HKMRCC is hkmrcc@mardep.gov.hk. Shipowner should submit the "ship movement report" 7, 5, 3, 2, 1 day(s) before the ship arriving the reporting line. 3. Shipowner is strongly advised that joining vessels should select their time of escort convoy according to the annouoced schedule and take the initiative to adjust their speed for the appropriate time of arrival. Shipowner should also submit at least 2 days in advance confirmation of the time of the escort convoy intended to join in order to avoid applying joining the escort convoy on arrival to the reporting line. Before sending the application, the masters and the shipowner should clearly confirm that they would join the navy escort. 4.
