Each master should be requested to set up a program whereby officers and crew will receive the maximum instruction in Fire Fighting and Emergency Life Saving Equipment. It is likewise necessary that all members of the vessel's complement, be safety conscious at all times so as to minimize the occurrence of accident casualties.

Such report should note the cause, nature and severity of the injuries and the resultant time lost. Moreover, the report should note, for some particular cases, the adequate steps or measures taken to prevent a further occurrence of same accident. The number of copies of report will vary from company to company, but one copy should be retained by the master.

Severely accident such as collision, the owners should emphasize that, neither the master nor any of his officers or crew should release any information or talk about the accident, except to the owner's representative or underwriter's representative or lawyer.
