I further certify that the casket contains only the remains of the deceased and on the basis of the embalmer's certificate hereto attached. The body appears to have been embalmed in accordance with Chinese quarantine regulation. and that a transit/export permit has been granted by the Chinese authorities concerned. The deceased Filipino seaman was a holder Philippine Passport No. kkkkkk issued in Manila on Feb/yy/2020. The remains of the deceased, which is inside a sealed casket. And placed in a box will be shipped by air to the Philippine, consigned to career, Philippine Management Inc. Suite 211, Aurora Bldge with the following Documents attached hereto

1 Death Certificate, and
2 Embalmer's Certificate, and
3 Transit/Export Permit.

Report on Store Lost

Dear Sir

        Please be informed that two coils of new coir rope measuring 10mm. in diameter and 60 meters in length, disappeared last night. Besides, two screwdrivers are missing too. The said coir ropes and screwdrivers were kept in the forward mast-house which was locked up properly by my boatswain last night but at about 0600 hours today was found open. After checking, the said items were found missing. Valued at about USD $800.00.

        I hope that the matter will be investigated promptly and that through your effort the missing items will be retrieved or their cost be compensated for.
