I further certify that the casket contains only the remains of the deceased and on the basis of the embalmer's certificate hereto attached. The body appears to have been embalmed in accordance with Chinese quarantine regulation. and that a transit/export permit has been granted by the Chinese authorities concerned. The deceased Filipino seaman was a holder Philippine Passport No. kkkkkk issued in Manila on Feb/yy/2020. The remains of the deceased, which is inside a sealed casket. And placed in a box will be shipped by air to the Philippine, consigned to career, Philippine Management Inc. Suite 211, Aurora Bldge with the following Documents attached hereto

1 Death Certificate, and
2 Embalmer's Certificate, and
3 Transit/Export Permit.
